- Same-day and urgent house calls are available based on clinical need and triage priority. For same-day requests, please call (305) 661-7609. Pepi Granat, MD and her assistant are avialable 24/7 for calls.
- After-hours triage: Available via phone for urgent concerns; for life‑threatening emergencies call 911.

Scheduling a home visit:
step-by-step

1) Call (305) 661-7609
Describe the reason for your visit — routine primary care, urgent house call, geriatric assessment, post-hospital visit, or same-day evaluation.
2) Intake information
Our staff will collect patient name, address, basic medical history, current medications, insurance details (if applicable), and any recent hospital discharge documentation. If you see any other doctor or have an interaction with a hospital or medical facility, please ask them to fax their reports to us at (305) 667-8266.
3) Triage and scheduling
Based on urgency and clinical need, we schedule the visit at a time that works for you. For urgent needs, we prioritize same-day visits when appropriate.
4) Visit preparation
All of your medications, including over-the-counter medications. If this is not possible, please provide a list that includes the drug name, dosage, quantity dispensed, number of refills ordered, and the prescribing doctor’s name.
5) Follow-up
After the visit, we will provide written instructions, schedule follow-ups, and coordinate labs or referrals as needed.
Insurance, billing, and payment
- We accept a range of insurance plans and will verify coverage when scheduling.
- For patients paying out-of-pocket, we provide transparent self-pay pricing and receipts for potential reimbursement.
- Medicare: Transitional care and home visit services may be covered depending on eligibility, medical necessity, and documentation. Patient charges are based on current Medicare rates, and Pepi Granat, MD generally uses in-office rates when appropriate.

- If you need to provide additional medical records or discharge summaries prior to a visit, you can bring them to the appointment or call the office to discuss secure transfer options.

Preparing for your visit — helpful checklist
